| Friends of Music Therapy Criteria for the Award: 1. Long term commitment to persons with special needs. 2. Demonstrates interest in innovative programming for such persons. 3. Has made a serious effort to understand music therapy and how it is utilized and practiced. 4. Has been actively involved with at least one music therapist in creating quality programming. 5. Has provided professional inspiration to music therapists, though example, motivation or partnership. Please include with the nomination the following information: Name, address, phone number of the person submitting the nomination Name of the nominee, current position, address, and phone number Length of association and description of initial connection with that person Description of the characteristics of the nominee as they relate to the criteria listed above Nominations must be submitted by: March 1st Submissions can be made electronically or sent by postal mail.
